Question:
Medical History- what if it is incomplete?
I have been reading that most insurance companies require a some kind of medical history from your doctor. I am one of those kinds of people who does not go to the doctor until I am ill. Most of my health concerns surround having PCOS which was diagnosed and has been treated by my GYN- I also had a very difficult pregnancy that was documented by my OB, perinatalist and dietican- my concern is not having documentation from a general family doctor- what are your thoughts- is this another insurance company decision? It depends on your insurance companies policy. I have Humana who required:
1. Med record showing I had been obese more than 3 years. (your GYN
records should work). 2. A letter from my doc stating they felt I would
benefit from WLS. (again, I would think your GYN could do this if they felt
comfortable doing so. 3. Documentation of a 6 month, physician directed,
weight loss effort. If your insurance requires that 3rd one you will have
to find a primary doc and do a 6 month program. Some insurances don't
require this, some require 3 months. I called my insurance company and
asked specifically then worked with my surgeons office. They were helpful
providing forms for my primary doc to use that Humana prefered. Getting
everything together including my psych eval took 3 months. When we finally
had the packet complete and submitted it to Humana I had approval in 2
days.
Preperation is the key so find out specifically what your INS company
wants, your GYN records could be enough or just a good start. Hang in

Boy, for years when I went to the doctor I always refeused to get on the
scales. This caused a problem when I wanted WLS because there was no record
of my weight fluctuations. I was able to find recorded weight when I went
for bone density tests. They need to know how much you weigh in order to do
the test properly. Lucky for me they still had their records!It's been two
yearsd post surgery for me and I've lost 140 pounds so I don't mind getting
on the scales now! Good luck to you on your journey.

You will need to get hooked up with a primary doctor. they should have
records form all your other doctors including weight. You may have to go to
your GYN and explain what you are planning so they include the weight in
your records.
