Medical History- what if it is incomplete?

I have been reading that most insurance companies require a some kind of medical history from your doctor. I am one of those kinds of people who does not go to the doctor until I am ill. Most of my health concerns surround having PCOS which was diagnosed and has been treated by my GYN- I also had a very difficult pregnancy that was documented by my OB, perinatalist and dietican- my concern is not having documentation from a general family doctor- what are your thoughts- is this another insurance company decision? Thanks
