Question:
Does anyone work for a government agency that is self insured
When they require certain things to qualify, do you have to go through it all again once you submit your packet. Right now WLS is not covered and they are going to add it to the plan but I have to submit a lot of info to them (my employer) first. Will the doctor have to request same info that i am providing already — bikermama (posted on February 15, 2008)

I work for a government agency that is self insured...however United Health
Care administers the plan, so everything I submitted was sent to them and
they dealt with it.
Although my approval process went pretty smoothly, one thing I learned
while going through the entire pre-surgical, surgical and post-surgical
process was to expect bumps in the road. So....my suggestion is that you
keep copies of everything you submit to everyone...that way if someone
needs something you've already submitted you've got copies and don't have
to go through finding and documenting everything over again.
The good thing about my situation was that my doctors office did ALL the
submitting of paperwork to my insurance..they told me what to give them and
they took it from there...so that really helped my approval process. If
you haven't found a provider yet, you may want to take things like that in
to consideration when you're looking.

Thank you for your answer. I too have an administrator but this surgery was
an exclusion under my plan. That is why I had to go through my employer so
they would put this into the plan. I just wondered if I will have to do
this again with the administrator once it is a covered benefit.
