Does anyone work for a government agency that is self insured

When they require certain things to qualify, do you have to go through it all again once you submit your packet. Right now WLS is not covered and they are going to add it to the plan but I have to submit a lot of info to them (my employer) first. Will the doctor have to request same info that i am providing already
