If I were you I would check with your insurance company to find out what
the requirements are. I know with mine I had to see my PCP for a referral
and then I saw the surgeon for the consult. After the consult I was
scheduled for the psyc exam and so on....There is a process for every
insurance provider. That would be a great place to start. Good Luck

Insurance companies usually want you to be at least 100 lbs overweight ,
tried several medically supervised weightloss programs, and have weight
related problems like arthritis, sleep apnea, etc. You didn't say if you
saw a surgeon.

After my PCP said she thought weight loss surgery was an option for me, I
found a dr. on my insurance plan got an appt. for consultation and then PCP
gave me a referral to go see barriatric surgeon. I then filled out about
12 pages of information, be very, very detailed with everything. Any co
morbities IE; acid reflus, high blood pressure, high colesterol so on.
Any family history of heart disease. Any diets you have been on over the
counter or perscription. Then all this was sent to insurance. I was
approved within two weeks with blue cross hmo of GA. I am now waiting to
go see psych. for evaluation and diet tech. for consultation. I also have
to have an upper gi and gallbladder scan. There is alot you have to do
before you even get a surgery date. Hope this helps.
