What happens now?

I have been approved by the doctor that I am in fact an obesed person. DDDUUUUHHHHH!!!!!! doesn't take rocet scientist to figure that one out. So he will submit his reccommendations that I need weight lose assistance. So what happens now. what do they do and what are my rights and can I just say I want the WLS or do Ihave to go through more diets. Please can someone help me. Thank you
