Call the customer service number on your card and ask them to send you
photocopies of those pages of the policy that specifically deal with this
surgery. I did, after I was denied. As soon as I read the details listed, I
knew why I had been denied.
I also used that information to put together my second request, making sure
every thing was in the same order as their check list from the policy and
so I did not miss anything.
8 months out from by-pass and down 137 lbs!!!!! That 2 yrs of waiting was
still the best thing I did to prepare & every pound is now worth
it!!!!
God Bless & Good Luck!!!

I have BC/BS of VA under my husband's policy that he's had for 30 years,
and yes, they did approve and pay for it with no questions asked.

I have BC/BS of NJ and had to wait a year before I could have the surgery.
They said that was 'cause my obesity was a pre-existing condition (DUH).
Once the year was up I was approved within 10 days of resubmitting the
request.

Christine, I have BC/BS of MA and I looked up the info regarding coverage
online. But, the surgeon's office can also tell you if you're covered or
not - they're used to dealing with this and responding to denials to help
you write an appeal letter if necessary. My surgeon has a person that does
this type of job, and that's all she does - maybe yours does to? Or if you
haven't picked a surgeon call around and ask. Requests are free - might as
well use the info! Good luck!
