My Dr. asked for the same thing and I do not know if it was for insurance
purposes or not, but I too had my points books and other literature that
they had given me. I had thrown away my weight books, who wants to look at
them when you are not on program!!! It just made me depressed. I would try
using what you have and also copy some cancled checks if you paid that way.
Good luck!

I would also recommend in addition to copy the books you have I would make
a diet chart listing as accurately as possible what you've done and how it
worked etc. Something like:
Weight Watchers 1998 lost 50 lbs regained 62
Slim Fast 1998 lost 12 lbs regained 10
Diet pills 1997 lost 8 lbs caused shakes had to quit
etc...

Get out your check book... make copies of cancelled checks used to pay
dues, or for diet books, groups, classes, lectures, diet pills,
supplements, etc., etc. Give them what they want and they can't say no.
Good luck to you!
