Question:
Did University Affiliates IPA ask for you for diet history documentation?

I would like to hear from anyone who was approved for WLS by University Affiliates IPA. I would like to know if they requested your diet history and if so, did they want actual documentation from the diet centers you went to? I am gearing up for a possible denial and want to be ready. I was asked to write down my history for Dr. Foster at Stanford University. I was not asked to prove it though. I was told by her that I was a good canidate. I am in process of getting apnea test. It would be hard for me to prove some of the diets I wrote down. Character witness's maybe?
