Did University Affiliates IPA ask for you for diet history documentation?

I would like to hear from anyone who was approved for WLS by University Affiliates IPA. I would like to know if they requested your diet history and if so, did they want actual documentation from the diet centers you went to? I am gearing up for a possible denial and want to be ready.
