Question:
Can anyone give me a link for, or a sample letter to ins. for the 6 mo. weight loss
My ins. needs a 6 mo. documented dr. supervised diet list and even though my dr. wrote some of the diets I was on in his previous letter, they want more. Is there a sample letter in the archives or can anyone give me suggestions? I actually haven't been to my PCP since last July and am going tomorrow so I need all my ducks in a row. what the insurance wants is for you to go to the doctor and start on a 6
month diet plan supervised by him.Some also require you see a nutrionalist
during this six months..Past diet histories are needed but this is not what
insurance is talking about..My advice..start the diet today ..with the dr.

I had to provide documentation of a doctor supervised diet too. What my
surgeon's office told me was to make an appointment with my PCP and tell
them I was joining Weight Watchers. Then, I joined and went to the weigh
in and meetings so they had proof I was going. After a few months, I went
back to my PCP to get weighed and so they could make a note in my chart so
it was technically being supervised by the doctor's office. I was approved
for surgery in January so I quit going to Weight Watchers then since I had
to save, save, save for my portion of the surgery fees (thank goodness for
annual out of pocket maximums!). I actually only attended weight watchers
from the end of August until the surgeons office submitted to my insurance
and it was approved in January. My insurance company approved my surgery
in a day with no questions asked.
