Can anyone give me a link for, or a sample letter to ins. for the 6 mo. weight loss

My ins. needs a 6 mo. documented dr. supervised diet list and even though my dr. wrote some of the diets I was on in his previous letter, they want more. Is there a sample letter in the archives or can anyone give me suggestions? I actually haven't been to my PCP since last July and am going tomorrow so I need all my ducks in a row.
