PLEASE help with examples of appeal letters. Already checked library

I am looking for examples of appeal letters for a breast lift as well as brachioplasty (sp?) (arm lift). I have BC/BS PPO, but Im hoping to get examples regardless of what insurance company you went through. Ive already successfully had my abdominoplasty approved and had the surgery in December 2003. However, now I need to work on getting my breast lift and arm lift approved and could really use your help. Ive already searched through the library database and unfortunately could not find exactly what I was looking for. Id TRULY APPRECIATE ANY HELP!
