PLEASE help with examples of appeal letters. Already checked library

I am looking for examples of appeal letters for a breast lift as well as brachioplasty (sp?) (arm lift). I have BC/BS PPO, but Im hoping to get examples regardless of what insurance company you went through. Ive already successfully had my abdominoplasty approved and had the surgery in December 2003. However, now I need to work on getting my breast lift and arm lift approved and could really use your help.
