Question:
I need to know my insurance codes
Someone had already answered this, but I lost my paperwork. I cannot find it in the library. Ok, my procedure codes are 43846 & 43659. I need to know exactly what they are and what they mean. I don't understand proimal and all that. Sorry for sounding stupid. :) Thanks for all of your help. — Red T. (posted on January 10, 2003)

My doctor posts this on his website. I hope it helps:
DOES MY INSURANCE COVER THIS PROCEDURE?
Most insurances do cover obesity procedures. The first step is to contact
your insurance carrier (the phone # is usually on the front of your card)
and give them the procedure code # CPT 43846, along with
diagnosis which is morbid obesity (ICD-9 #278.01). Be
sure to get the first name and last initial of the person you spoke with.
They will give you the requirements to meet their "medical
necessity". The average insurance company requires documentation of at
least two failed medically supervised diets. This includes letters from the
supervising physicians and their medical records.

43846: Gastric restrictive procedure, with gastric bypass for morbid
obesity (dx 278.01); with short limb (less than 100cm) roux-en-y
gastrotenterostomy.
43659: laparoscopy procedure of the stomach.
I work for an insurance company and this is what we have for these in our
system. Hope it helps!
