I need to file 2nd appeal and need help (samples of letters etc.)
Can anyone please help me with an internal appeal letter to send to PHS. They are denying me due to lack of 2 yr diet history. Have just over 1 year with pcp and 18 month with OBGYN (on restricted weight gain with preg)Please help. Thanks. — JERSEYGIRL723 (posted on March 14, 2002)

Do you have any diet history with previous doctors? If so, you can get
your records from them. If you sign a release with your current PCP and
they ask for the records, it won't cost you anything. If not, does it have
to be physician-supervised weight loss attempts? If so, you may not have
grounds for an appeal. And, if you only need 6 more months, you could have
your doctor put you on a diet and then reapply in 6 months. If the 2 years
of diet history is clearly spelled out in your policy and you don't have
it, your appeal is unlikely to be successful, but 6 more months would be
worth it, at least to me.

I would have to agree with the other poster .... the insurance companies
have very strict guidelines they follow in order to determine your claim.
If they require a certain length of time on supervised diets, you have to
have documentation of that, even if you've been on a million
"unsupervised" diets before. I agree ... you've only got a few
more months and you'll qualify. Just have your doctor put you on a diet
and document that he/she has done it. Good luck! I'm sure glad my
insurance company didn't require all that!!!!
