I need to file 2nd appeal and need help (samples of letters etc.)

Can anyone please help me with an internal appeal letter to send to PHS. They are denying me due to lack of 2 yr diet history. Have just over 1 year with pcp and 18 month with OBGYN (on restricted weight gain with preg)Please help. Thanks.
