Question:
Ins co tells me with 4 weeks left i need 2 yr. sup diet. HELP
After 6 weeks PHS decides to tell me know i need psy eval and 2 yr sup. diet. I made appt for psy eval and to see my doctor who super my diets 2 years ago for 4 months on prescription meds. Are insurance companys very strict when it comes to going to doctor every month/s or can they write a letter stating my sup diets and then say that i was on so and so (TOOO MANY TO MENTION) from that time period until this visit with no success. PLEASE HELP I am scheduled 2/14/02 and have made too many arrangements to have surgery rescheduled. THANKS — JERSEYGIRL723 (posted on January 15, 2002)

I supplied a notarized affidative from a friend I was in Weight Watchers
with and I had gone to a diet doctor who had recorded I was in WW. I only
went to him a few months but did a followup a year later together it was
enough to meet Anthems 18 month supervised diet requirement.

I think it depends on the insurance co. I sent in actual copies of pages
of my chart from my PCP. I had been on Redux, Phen-fen,Meridia over the
course of about 2-3 years.
I actually lot weigh on these. But you know..... I gained
it all back plus some! I was approved in 24 hours! I also copied a letter
from someone on this site and just added my
own co-morbids, and my PCP signed it. (she's very supportive for a 110#
lady!) This is all i needed to get approved. Some insurance's will approve
you w/ BMI 35 plus comorbids. Good Luck

Hi Kathleen, I dont know if this will help. My insurance wanted 3 year
history. I got all my medical records and had the surgeons nurse use all
the medically documented weights that I had documented. She said that was
all she needed!!!! Heck, everytime I went to the doc we talked about my
weight... so that makes a 30 year history right there... I was approved in
2 days.. and I think those two days were Sat and Sun!!! LOL.... Good luck
