Ins co tells me with 4 weeks left i need 2 yr. sup diet. HELP

After 6 weeks PHS decides to tell me know i need psy eval and 2 yr sup. diet. I made appt for psy eval and to see my doctor who super my diets 2 years ago for 4 months on prescription meds. Are insurance companys very strict when it comes to going to doctor every month/s or can they write a letter stating my sup diets and then say that i was on so and so (TOOO MANY TO MENTION) from that time period until this visit with no success. PLEASE HELP I am scheduled 2/14/02 and have made too many arrangements to have surgery rescheduled. THANKS
