MELISSA~~ HERE'S A FEW IDEAS~~ Gather up all previous medical records if they are available. If you have them in your hand when you arrive, it could speed the process......Write a list of all of your dieting attempts, medically supervised or not. Be sure and notate how long you were on the diet, how much weight you lost and how long you kept it off.

Hi Melissa - why don't you call doc and see if you need to bring anything to speed up the process, My doc had a list they sent me. I needed a letter of medical necessity from my PCP. Good luck. I am post 4 months and down 90 pounds (70 from surgery)It was so worth it for me!
