Does anyone have a suggestion for appealing a denial letter from bcbs bluecard PPO

What is the reason for denial. I need more info to try and give you some help. I have won a claim appeal with BC/BS before, not for anything related to WLS, but do have some knowledge that might of help if I know why it is being denied.
