I'm seven years out....and know my body and understand nutrition better than most. It was hideous because at those numbers I was very sick. A diet rich in iron (and oral supplements) will accomplish nothing if the portion of your intestine that absorbs it is missing or no longer functional...people need to understand that, especially medical professionals like yourself.

Three years ago I was a beautiful fat woman who liked herself but had bad sleep apnea and was damn near immobile (at 370 pounds). I had blood sugar (dumping type) issues and gastric problems. I had the surgery because my quality of life was getting to be badly compromised.
I am now a size 10 woman who had to spend $33k on plastics to remove the tremendous skin piles, be treated for a Vit D deficiency, had and emergency gallbladder removal, and emergency bowel obstruction (which nearly killed me.) I have chronic bowel impactions and constipation. I have half the red blood cells I had just three years ago and the anemia's making it hard to get moving, and the treatment is making me gain weight. I have to take daily hormone replacements for the huge ampunt of estrogen that's been dumped in my system. I have to count literally every thing that goes in my mouth and balance it all nutritionally. My groceries cost twice what they once did. I spent over $400 this month on insuranec co-pays alone.
So my answer is this: if I get to live a longer life, it will have been worth it. And if this mess kills me off, I will be highly pissed. I said, "I will live free or die trying" and only time will tell where I get to fall on that continuum.
If you are not in the "I will live free or die trying" place, I wouldn't do it. If your health is not compromised, I wouldn't do it. If you think it will solve all your problems, I wouldn't do it. I knew these things were a possibility and had to take a shot to save myself-- no other real options, so I am sort of stuck dancing with "the one that brung me" for both the good and bad of it.

I used to read about that stuff and think, "Well, that stupid slob didn't take their vitamins. Duh."
I have taken my supplements religiously. More than most people, and following all the rules. I have been accountable every day, balanced my diet, and now I am tanking once again.
This time, it's anemia. I started seeing a cardiologist in April because I kept "hitting the wall" when I exercise-- it was suddenly getting harder, and wearing me out. He discovered a new murmur, but nothing major.
In May, I had my hormones tested, because I was so emotionally labile and weepy I wasn't making any sense. We discovered I had three times the amount of estrogen in my system of a normal woman my age, so I started taking HRT. And it was through these labs that we also discovered I am anemic. Which explains why I just haven't been able to get my head up off the floor, bootstrapping or not.
My Ferritin level is a 9. (Normal is 13-50.) Your ferritin is your back-up or storage iron. Once you start depleting that, you're hosed. My red blood calls (normal is 3.8-5.1) went from a 4.3 to 2.9. My iron saturation (nromal is 15-55) is at a freakin' 4.
So my PCP tells me to take some more Carbonyl. Mmmmmkay-- but of course it did not a damn thing. That's the thing with the iron-- you either absorb it orally or you don't. Turns out I don't, and had been living off the stored stuff for three years. So we toussle a bit and he sends me t o a hematologist/oncologist. I watched my labs tank over time, but no one would do anything because the weren't officially *low.*
In 2008, my Ferritin (stored iron) was 149 . Again, today-- it's a 9. NINE. Now, I am *officially* LOW.
I go to the hematologist, and he says, "No, that's NOT normal, that's hideous. We can give you some better iron, if you think that will help." By better iron, I know he means heme iron, which costs me $70 a month, and I have figured out by now will not do squatola to raise iron scores. So I say, "Let's skip the formalities and talk infusion." And he says, "Bigger issue is why you're losing the iron, but yep, I agree, you need the infusion, and you need it now. Like, today. Like, let's go now."

The infusion lab is also the chemo lab. You immediately are treated very very kindly ("Care for a drink? A cookie?") because the people around you are fighting for their lives. This is humbling. They even offer you a special parking spot, but I refused mine-- as long as I am able-bodied, I am hoofin' it!
I was lucky-- I am getting Venofer. Venofer is an iron sucrose IV. The first infusion lasts about an hour and they watch you very closely for anaphylaxis. I felt fine. I am scheduled for nine more shorter infusions-- each lasting a half hour. (I have now had three so far.) The doc tells me we will retest in a while, but there's a very good chance I will be coming to this blood lab three times a week, cyclically, for the rest of my life.
I had the RNY to get healthy. Truly. And this ain't it. So yep, while I appreciate the smaller size, these sicknesses make me angry. And yes, I am thrilled to have insurance, but the $30 co-pay each time I walk in a doctor's door (and that's 18 times this month) is kicking my ass up and down a street. And while I am so very glad not to be one of the chemo people, it sure is a freakin' bummer to literally spend my entire summer in medical and surgical care, as a direct result of an electove surgery I had on a perfectly functional gastointestinal system.
My friend said, "But hey, you look great." I wanted to kick her ass on the spot. If I end up a gorgeous corpse, I am going to be so effin' pissed.

For those of you who have gone the infusion route:
1) Did you get the level back up and maintain with oral iron, or are you back in for infusions periodically?
2) You get yours in big doses, or smaller ones spaced days apart?
3) Any side effects? (I feel like I wa**** by a bus the day after, just general malaise.)
4) I am noticing a little weight gain-- did you have that as well? How much? Did it come back off after you stopped getting the IV fluids? (I know, it's 30% sucrose and a bag of fluids, so water gain is unavoidable. I don't want to eat less, though, because I feel so weak as it is. SHould I drop back my calories?)
5) How low were your numbers, and did you continue to exercise? Any changes in your exercise routine?

Sig Ot and I are doing well. I'm still up and down between 149-157 pounds, (am 153 today) over and over-- I retain buckets of water when I exercise and I still do Zumba 2x a week and try to walk most days. I still count every calorie M-F, (I eat 1700 a day) and am looser on weekends but not crazy-go-nuts, and eat like it's going out of style on holidays. No regain to speak of, and I feel good and clothes still fit well over 3 years after I started losing, so all's well inna world, I suppose!
Still love cooking-- the guy who was helping me with my website crapped out but I'll get on it when I can-- I have recipes galore (I just make a chicken alfredo caeerole that's mindblowing.) My meals tend to be about 200 cals for breakfast, 300 for lunch, 500 or more for dinner, with plenty of snacks-- I eat an apple every fweakin' day because I love apples, and end most nights with a whole cup of greek yogurt and tbsp of Dark Chocolate Dreams peanut butter, which is about the world's most perfect food. I have ice cream in one form or another about 4 times a week in the summer, and it works for me. So the post RNY lifestyle doesn't have to be a death March: it's about management, not deprivation. Eat smarter, not less.
My most recent pic is quasi-racy because I wanted people to have a sense of what's achievable with work, a plastic surgeon and a little moxie so I done dismembered m'self to show it-- I have 2 more rounds of plastics to endure (my ass is heinous and will be done this summer, and thighs when I win the lotto.) You lose 220 pounds, you're gonna have skin-- that's just the name of that game. I've had my face, chest (no implants), gut and arms reconstructed already. Well, and my hair, which came back inexplicably curly after my last 13 hour surgery!

ETA: To answer your questions:
There's no secret-- I eat less than I spend (and monitor that very closely) and I keep a balance of about 35% carbs/ 35% protein / 30% fat and 1700 calories a day-- with the exception of holidays, where I eat as much as I want of whatever I like.
I'm up .8 lb from my all-time low, so no, no real regain.

If you are suffering an intestinal blockage, all the noise in the world may not save your life. Or better said, if you think you're being loud enough, BE LOUDER-- and arm yourself with knowledge so you recognize it beforehand.
My story: 3 days after having my gallbladder out in June, I started feeling very nauseous and suddenly could not keep food down. (That'd be hard for some to note because they have trouble keeping food down anyway.) I hadn't had gas or a bowel movement in 7 days, but having just had surgery suspected it was just slow in coming.
After 6 hours of this nausea and pain not passing I paged my surgeon and his assistant. I was told that neither were answering their pages. I was able to get in some water eventually and went to bed.
The next day the nausea and pain intensified. By 6pm I could take no more. I was home alone, and so instead of driving to the metro hospital where I had my RNY, I could only drive as far as the local ER. This choice almost cost me my life.
I was sweating and crying when I went in. I explained to the triage nurse that I had an RNY and a gallbladder removal just days before and suspected I had an intestinal blockage, and that the pain was excrutiating. An emergency. I could die.
But It was a holiday weekend, and people who cut their fingers on beer pull tabs were taken back, women who were upset over boyfriends, the general ER riffraff BS that takes place on a holiday-- and I was left standing (couldn't sit) in the waiting room for FIVE hours-- even though I returned several times to triage to say, "I can't take this much longer-- I am very afraid I have had a blockage and need a doctor NOW."
When I was finally seen, the CAT scan was "inconclusive"...the doc told me he saw a lot of fluid in my belly but thought it might be from the gallbladder surgery, and I should follow up with my surgeon in the morning. I was discharged at 6 am with nausea meds and pain pills.
When I called my surgeon, I read him all my tests-- enzyme levels, etc. He said he didn't hear anything alarming-- but he was listening for something gallbladder related-- a bile duct blockage etc. would skew the numbers. When it didn't, he more or less said, "You'll be fine." This was Monday.
By Wednesday the pain at night was unbearable. I cried and screamed into a towel. Wednesday morning I had a regular check up with my surgeon. Luckily, his assistant knows me very well and said, "Shari, you don't look right. Seriously." The surgeon said, "I don't know what to do-- want us to admit you and run some tests?" I think he expected me to say no, but I said, "PLEASE!"
So I was admitted around 4 pm. By 7 pm the pain was back, but they wouldn't allow me pain meds until after I had a CAT scan. I didn't hve the CAT scan until 3 am. They left me on a gurney in the hallway for an hour and a half aftyerward, sobbing in pain, because they didn't have anyone in transportation to come get me. By the time I finally received pain meds at 5 am, I would have shot myself if I'd had access to a gun.
I finally fell asleep. At 7 am, my surgeon breezed in, threw open my curtains and said, "You're having surgery today. You have a bunch of fluid backed up in your belly, and we need to find out why." He explained he'd use the lap incisions he'd made for the gallbladder and go back in.
By noon I was in surgery.
At 4 pm, I awoke in the worst pain I had ever felt in my life. I focused on a spot on the ceiling. I could not talk. I had tears streaming . The dilaudid wasn't even touching it.
They explained to me, "You're a lucky woman. Your bowel (intestines) had twisted and actually permeated the wall of your excluded stomach. Generally when this happens, and days pass, we find a bunch of dead bowel which we have to remove. But for whatever reason when we reconnected you, you pinked up right away. Now we just have to see if it works-- if you can move your bowels or pass any gas." I was told that the lap procedure went open-- suddenly-- when they couldn't locate the source right away-- I had a large open incision now from my sternum to my navel. I'd been gutted and beat up pretty badly in the process. But I was alive.
For five days, nothing passed. My urine was dark brown, I was losing kidney function. Then it started to lighten, and returned. And finally on the fifth day, while walking in the hall, I passed gas. Sounds stupid, but I was so happy I cried. I gained 25 pounds in fluid and unpassed stool in 3 days--but within two weeks lost 40. My recovery took many weeks, but I finally feel better now. I am grateful for the opportunity to live, and feel obligated to warn anyone who will listen.
Blockages like mine are 100% fatal if not caught in time-- usually within 72 hours. Why mine didn't take me down in the FIVE days that had elapsed, I am not sure.
I wanted to make sure you know what we're really discussing-- how it feels, what it looks like, how you might be treated. Your local ER will NOT be able to help you. You need to educate loved ones so that they can speak FOR you-- a medical alert bracelet will not help you. The beloved poster who passed away may have done all of the above, and still they missed her blockage until it was too late. You need to do everything you can to make sure it doesn't happen to you-- and this sort of herniation-- sometimes called a Petersen's hernia-- happens in about 5% of post bypass patients. Five out of one hundred. One in twenty. Could be anyone.
Symptoms in an RNYer include nausea, vomiting, abdominal pain and distention, inability to pass stools or gas. (Apparently in DSers, gas will still pass.)

Your body takes calcium from your bones when it's in crisis and believes it needs to...so you can have a "high" calcium level even if you aren't supplementing enough, if you know what I am saying.
A high PTH means that your body is not absorbing your calcium supplements. The above posters were well meaning but incorrect-- there IS something you can do.
First-- make sure you're getting 1600--2000mg elemental calcium citrate daily. Then, you add large supplementary doses of dry Vitamin D3-- and by large, I mean LARGE-- not 1,000 iu a day-- more like 50,000 iu a day. Take that for a couple months, run labs again, and see where you are. (In my case I now take it 3x a week.)
Technically, too much vitamin D can be toxic but if you are malabsorbing it-- as it appears you are-- then it would be no big deal. You really need a specialist to confirm this.
Vitalady.com sells the large D3 doses-- they are otherwise very hard to find.

I didn't have a cosmetic surgery or to prevent any hypothetical or "overexaggerated risks." I came to this surgery as an attractive, accomplished, self actualized, aware and intelligent fat woman who could not longer tolerate the pain and associated illnesses my obesity was directly causing me. I was not hoodwinked, bamboozled, mislead, manipulated, or sold anything. I did my own research and found a potential solution to excrutiating pain, immobility and illness, and went with the best solution I could identify.
To suggest or even imply by making such a generalized statement as yours ("Theyll just sign" et al) is shortsighted, pompous, and insulting.
