Many, many thanks for not only posting this, but for also supplying the link. I have a friend who has met the requirements for WLS, but our BCBS of TN does not cover the sleeve. I've printed this off to help her with her appeal. If she gets it, I 'll tell her has to name her next cat after you. She collects them (but no more than she can afford to take care of
