Question:
What should I send along with my letter of medical necessity?

I have this letter from my GP that I posted earlier. Should I send that alone and then send the letters from my ob/gyn and orthopedica surgeon if they deny me, or should I send the whole packet with my personal letter, letter from GP, letter from ob/gyn, letter from orthopedic surgeon, all information I have gathered to enlighten them on the procedure? My HR department says that I shouldn't have a problem getting approved - even though we have an exclusion in our policy that specifically excludes this type of surgery (and I reminded her of that) but she said if they do to appeal it and then send it to them and they will consider paying for it. So I am also preparing a cost analysis - should I go to a urologist about the incontinence, and a vein surgeon about my awful varicose viens in my legs as well?    — K T. (posted on March 18, 2001)
