Question:
Where can I find the Ideal Body weight chart used to determine 100+ lb weight loss?

Barb, Each insurance company has it's own tables, but one of the more
popular ones is the Metlife table. It can be found at:
http://www.metlife2000.org/Lifeadvice/Tools/Heightnweight/Docs/women.html.
This is a good guideline, but if you are cutting it close, you should ask
your insurance company for its table. My insurance company actually used a
scale that said at 5 feet you should weigh 100 pounds and then add five
pounds for every inch above 5 feet. (This was the small frame scale.) I'm
5'1" and I'll never weigh 105 pounds, but the low scale helped me
qualify. Good Luck!
