Question:
Has anyone heard of Health Max

my ins co lists it's main name as Plaines Health Network, then I find Accountable Health Network as the pre-cert co.Plan supervisor is NBR Admiistrators. Plan Name-GPCManagement ,Inc. I am totally confused,then read further that it is a self-funded health plan,administration thru a 3rd party claims supervisor. also, obesity care and treatment is listed as an exclusion, so I am getting more and more depressed/discouraged. I have an appt with the surgeon tomorrow and then they will submit to the ins co.-I was just wondering if anyone has heard of this co or dealt with them..thanks    — Norma N.
