Need 2 make sure I'm using correct insurance code on my letter
Oops! Didn't get all my question in on the previous post! Here it is: For initial request from my insurance, I'm using the letter provided in the "Your Journey" section of this site, but I believe the procedure code listed there is for OPEN Gastric Bypass. My surgeon is wanting to do LAP on me, so it would look pretty dumb to my insurance if I didn't have the right code on my request. Can someone confirm that LAP procedure code is 43659? P.S. - Others that are using this letter as a guide might double check to make sure they have the correct procedure code! Thanks! — Carly H. (posted on September 30, 2003)

Does your ins. require YOU to send the request? Most surgeons do this
themselves.

Original poster here - My insurance asks for (in addition to 5 yr. medical,
diet & exercise history) a cover letter that includes my co-morbidities
and states medical necessity. I am writing and having my primary care
physician review and sign. I'm not sure exactly what my surgeon turns in.
The codes will be on this letter and I have used the letter on this site as
a guide and personalized it to fit my situation. Copme to think of it,
maybe I could leave off the references to procedure codes - surely my
surgeon's office will give them that. Thanks.

Procedure Code 43847 is what you are looking for. Lap Gastric Bypass (RNY).
