Question:
My Primary Care Dr. needs to write a letter (himself) for my ins.
I've read the sample letters on the site, but my insurance requires a Letter of Medical Necessity, written by my DOCTOR, not me (although I have my lists of co-morbidities and diet history). Is there a sample of a letter I can give to my Dr., for him to go by (if he chooses to) in composing HIS letter to the insurance company for me? He's very busy and may appreciate this If anyone knows of a sample letter I can provide him with, I'd appreciate it soooo much! Thanks! — Carly H. (posted on September 15, 2003)

Hi. I got the letter of medical necessity from this site and changed it to
fit my situation. I took it to my Dr. for reference. He said he needed to
write the letter himself but signed the one that I printed from this site.
