Hi...I have never been on a doctor-supervised diet, but went on every other
diet known to man. I documented all of those (weight watchers, T.O.P.S.,
Slim Fast, etc.), how much I spent on each, how much weight I lost and
gained back. My insurance company, luckily didn't require all of that.
They just wanted proof of BMI of 40 or more, and the surgeon's
recommendation. I consider myself lucky and am happily 4 weeks post-op,
down 26 pounds! Hang in there, put as much documentation together as you
can, and be diligent when getting approval! Call often, help them to help
you, and be polite!! Good luck to you!!

Check your doctor see if they have been keeping records of your weight each
time that you go in to see them, if they have been doing so you can get
copies of these notes from you doctor and compile a Diet history letter
along with these notes showing the trend for the last 12 months.Make sure
that you State in your medical necessity letter from your PCP that you have
been on a low carbs high protein diet for the last 12 months if thats what
your insurance requires and attach the medical the notes with the diet
history letter to it.
