Question:
The surgeon's office is giving me the runaround
I have already been approved and my surgery date is set. So those hurdles are over. I mailed the forms my employer needs filled out (FMLA form and short term disability form)to the surgeon 3 weeks ago. I even included a self addressed envelope that was stamped. I addressed it exactly as I was told to, and put it to the correct person's attention. I called yesterday and they acted like i was speaking a foreign language. Can't recall seeing it....Must be around here somewhere...I'll look for it when I get a chance. I'll do my best to call you back. they never called me back. She acted like this was no big deal. I have to have the forms back to my employer by Oct 1 as my last day at work is Oct 9. The office is 2 1/2 hours away, so I can't pop in and give it to them and wait for it. This office has never once returned any of my calls, I always have to call them back. Any suggestions on how to handle it as I got the impression she was not going to go out of her way to resolve it. — claire C. (posted on September 14, 2000)

I faxed mine to my surgeon's office. On the cover I was very specific
about when I needed it back by, and I gave them the fax number to send it
back too. Try that and see if it works. If you don't have access to a fax,
perhaps there is a Kinkos or something like that where you could fax it
from. Good Luck!

I had the same problem and I switched surgeons. But you have a date and I
would hate to start over again. Have you tried speaking with the surgeons
assistant or getting a message to the surgeon. Wish I could be of more
help.
