Some states do have a law that requires insurance companies to cover WLS if
medically necessary. Now, the sticky part is, that your employer is under
no obligation to buy that part of the policy. So while it has to be offered
by the ins. companies, the customer (your employer) is under NO obligation
to buy.

In addition, in states like Maryland if an employer is "self
insured" or does not have more than a certain number of employees,
they do not have to cover it either.

I think maybe you were wanting to say "medically necessary" - is
that right?
Sadly, not true. I was dying, had letters of medical necessity from five
physicians, and was denied by TWO different insurance companies.
So I self-paid. There ARE options if insurance doesn't come through for
you. Let me know if I can help at all, okay?
