Question:
Does anyone have a template for the letter?
I have almost no medical history (I've only been to the doc twice in the past 15 years and those were both in the last 2 years), and no diagnosed comorbidities. I have a family history of diabetes and heart disease, so I know I'll develop them in time, but I'd like to get the problem fixed pre-emptively. Has anyone in a similar situation been approved, and if so, is there a copy of the letter I could use as a template for my doctor? — mbausch (posted on May 20, 2003)

Hi Matthew, Have you looked at the letter of medical necessity under the
"Your Journey" section on this website? I copied the whole thing
to Microsoft Word and changed it so that it pertained to me. I found that
it helped remind me of medical problems I have that I may not have included
otherwise. And not all of these problems were documented in my medical
records. Good luck.

I'd get your insurance company to spell out exactly what the authorization
requirements are for the surgery. My insurance doesn't even require
comorbidities at all - they have plenty of other rules, but cormorbs is not
one of them.
