Question:
Has anyone gotton an approval from Md. Fidelity without any record of supervised diet

I am going to have gastric bypass and a girl in my office is also. Our insurance told her today that she needs a medical history of supervised diets or receipts to show she has dieted on her own. Is this true? Has anyone been approved with Md. Fidelity without. Any help would be appreciated. I will have to deal with this shortly and want to be prepared. Thanks!!    — donna B.
