Question:
What should I write to get approval the first time around

I called my surgeon's office to set up a date for surgery. The secretary told me that my psychologist, regular physician, and I would have to write letters to add to the surgeon's to gain approval from MN Medical Assistance. Does anyone have a really good example so I can get this approval the first time and right away?
