Question:
What do I need to do for the grievance committee?

i have already sent in pre approval -denied i have sent in appeal- it will be reviewby the grievance committiee on dec. 13 what do i need to send to them for this.    — julie S. (posted on November 3, 2001)

I won after a teleconference call with greivance committee... I can email you my notes OR you can find them through a trail of my profiles. I'll sign on with the certain profile that has the notes in it. I went detail by detail of the processes that I went through... the criteria, how I met it, etc. Send them anything and everything that proves that you need the surgery and that it would be in their best interest. Copy of receipts for medications, chiropractor, counseling, physical therapy, gym membership, diet history, weight records over last five years or so from doctors office showing that you've been obese for so long...etc. My second appeal was over 30 pages of stuff.
