Hi Sharon, check out member K Taylor's profile. She has one posted there. I will find the other's I have seen and email you the link. There also is a company that for about 30 bucks or so writes one for you I also have their email. LMK if you'd like it. I have Kaiser San Diego just got denied. What they need is more clarifications of my co-mo's so I am having the PCP redo that and resubmit. They told me it would fly then.Good Luck.
