Question:
What information should I include in my cover letter for my insurance company?

I will be sending in a cover letter along with my doctor's findings and support, but I don't know how to begin or what information to put in it. I will be sending it in around August because I am not actually a member of the insurance company yet. If I get approved, I will be switching to that insurance in October/November.
