I think with medicaid Your Physician is supposed to write a referal to a
surgeon (Of your choice) and and then you will need a 6 month doctor
supervised diet. Basically they need previous diet attempts and proof of
them. It isn't to bad your surgeon will know what to do. Best Wishes

Hi there, I had medicaid and medicare and live in Iowa......had my Ds/bpd
done in Mpls MN.
When I met with my surgeon for a consult......he asked medical history and
then he sent a letter to medicaid/medicare explaining my health problems
and how he feels they would be better after wls.
They do NOT pre qualify.....all that happens is that your doc will submet a
letter of necessity and it will be covered automatically. No
worries......you are fine as long as you have at least 2 health problems
related to obesity. I had (have) thyroid disease, depression/anxiety
disorder and lymphoedema, sleep apnea and border line diabetic so I was set
enough for 2 people lol
Good luck! :)

I have Virginia Medicaid and it covered my entire surgery. I didn't have
to file anything. The surgeon's office took care of that.

I am in California, and MediCal paid for my surgery. I had a BMI of 39.1,
high blood pressure, Gerd, Arthritis, and other comorbidies. All I did was
fill out my physician groups questioniere, listed all of the ways in which
I had earlier tried to lose weight, and my relitives health history
pertaining to problems caused by obisity. Hope this helps. Make sure you
get a good group, with a competent insurance person!

I just had my surgery on Thursday the 20th. I have medicaid of NJ. Without
an HMO, they said that tere is no pre authorization. It is automatically
approved. I would look into it in your state.
