I have BCBS and they require a 12 month dr supervised diet history--which
they do not pay for--they will send you a letter to send in all dr and non
dr supervised diets---I hadd 14 months of diet history from Metabolic
Research ad they rejected it as there was not doctor on premises to
examine me every time---I do not see how they can require you to
participate ina dr supervised diet that they will not cover at any
percentage--I have hired Walter to represent me---this has been going on
since Nov. 01--I was just denied for not having supporting evidence---they
have 14 years of medical records---its just a game with them

I had a friend who did not have a lot of history either. I told her to find
anything and everthing she could pertaining to her weight, and diets,
including ANY healthclub memberships she has been a member of, even letters
from friens/family. While they may not be med. supervised, it worked for
her.OBGYN, Even chiropratic treatments do to back pain, etc...You may have
more than you realize if you go back far enough. Good luck!
