Question:
Would like to receive sample appeal letters from anyone who has Aetna HMO?

I know I might not be of any help to you . I just wanted to tell you to find out if your insurance has a clause that they exclude any weight loss surgery.I have Aetna HMO patriort too. I live in Georgia and they do have the clause in their policy that they do not cover weight loss surgery. You may be under a different clauses, I understand that it is totally up to the company you work for if they opt to have this included. My answer to you would be to call member service and ask if they have ths exclusion clause ,then I would proceed to appeal if they don"t have it stated in their policy.
