Question:
Does anyone have this exact insurance?

Teresa, Have you tried calling your insurance company and speaking to them directly? I had some questions about my insurance because I work for Walmart (which is notorious for denying weight loss surgery for it's employees) but, when I first chose my insurance 2 years ago, I opted for an HMO option rather than basic Walmart insurance. I knew that the HMO in general covered the surgery but was concerned because I kept hearing that Walmart excluded it in it's policy's so I called them directly. If you don't feel comfortable, see if they'll give you any answers without giving your name and policy number, etc. Good luck with getting approval.
