If your company specifically states they do not cover WLS I don't know that
you have any options. I was immediately approved & I have United
Healthcare Select Plus POS.

I have United too. The only reason United won't pay is if your employer
does not want to cover it in its policy. You can appeal it but there isnt
much they can do if your policy owner does want it coverd. Also on every
hard copy poilcy they state they will not cover WLS, but in fact they do.
Call and ask if it is covered if it is not ask if it is the ploicy owner
that not wanting it covered. If you want to appeal to your HR dept of your
employer. Good luck

I can't help you with an appeals letter, but I DO have a suggestion. Go to
http://www.obesitylaw.com and when the "home" page comes up,
scroll to the bottom where you'll see the different "sections" of
the site listed. Click on "articles". Some GREAT information
here on how to do battle with insurance companies.
