Question:
I am going to be mailing my request tomorrow, is there anything else I need ?
I will have medical records (comorbities & weights highlighted), a letter of request from myself, a diet history (Dr supervised & not), and pictures of myself. My PCP has already submitted her letter of recommendation so that is done already. can you think of anything else that may be helpful? — Lynda T. (posted on June 26, 2001)

Hi Lynda, I'm not sure if you mean that you are mailing the request to the
ins co or your surgeon's office. All the info needs to be sent from your
surgeon's office because the doctor also needs to submit a letter of
request on your behalf, along with the procedure and diagnosis codes. So I
would not advice you to send it to the ins co if that's your plan. Also if
your PCP sent the letter to the ins co, I would suggest that you get a copy
of it to attach to whatever you already have and send everything at the
same time. It's not wise to separate info, because then it's possible that
the ins co will misplace something and ask for it to be sent again. What
you already have may be sufficient, but find out if you need a psych eval.
Also if you have life threatening comorbs then it would be a good thing to
have the relevant specialists write letters on your behalf to recommend
the surgery. I wish you the best.

I wrote a letter to my insurance even before my Doctor did and I told them
my weight, height and BMI and I attached the diagonsis(sp)of what co-morbs
I had and told them to be expexting a letter from my Doctor. I am still in
the waiting process, but I know my file is before the review board. I felt
like I need to do something. Good luck to you!
