What should I take to my INITIAL CONSULTATION to speed up the approval process?

Hi! My initial consultation is 2/7/01, and I'm so excited to FINALLY get the ball rolling! NOW, I want to be sure that 'Ball' stays in motion! Any suggestions of what info I should take with me to the first visit? I have Guardian PHCS PPO. (ie: medical records, list of diet attempts.... WHAT ELSE???) Thanks for your help!!!!    — Laurie L.
