Where can I find a sample appeal letter?

Hi, Everyone. I had my surgery 9 months ago and it's the absolute best thing I could have ever done for myself. Seeing me get the surgery inspired my sister to make the decision to go for it too. Here's the problem Blue Shield HMO denied her on friday. I felt so bad for her. I remember what it felt like while I was waiting to get approved. She is so busy with her kids and family that I want to volunteer and help her appeal the insurers decision. I went to the library here and searched for sample appeal letters. A lot of people reference to a sample letter here on the site. I have looked everywhere and can't find it. Can some please help me? Be sure you address the reason she was denied. Sending an emotional "but I NEED this surgery" letter won't do any good, in most cases. You need to be specific.
