Question:
My insurance company wants a detailed weight loss , diet history.
Does this mean they want it from me or from my PCP? They didn't state from PCP - just "detailed weight loss, diet history." I can write up the most detailed diet and weight loss history they can get and be honest about it. — Sonya P. (posted on June 3, 2003)

i filled out a three page diet history. Names, dates as near possible,
success. My pcp was not required to do that. It was part of my medical
history that the surgeon took to send to the insurance company. This may
vary with insurance companies.

A lot of insurance companies require the diet history - it needs to be
detailed, dates from when to when, name of diet, how long you were on it,
how much lost (if any). This needs to come from you - if you went on a
medically supervised diet by your PCP - get copies of it. You need to have
as much information as possible. I even had to get 5 years of medical
records! Good luck!

Sonya, do both if you can. I had 2 pages of diets and gym memberships and
exercise videos, etc. I also had a letter written by my PCP exlaining how
this surgery would help me. I never had supervised diet perse, but he did
say I was under his care while I tried traditional diets. I think he helped
a great deal in my approval, so do both if you can! Goodluck to you :-)

how about this, i do a detailed diet history and get a letter from my PCP
stating I was under his care (which I was and he knows it) while I was on
this list of diets

Hi. I had to do this too. My surgeon had a form to fill out and then I also
faxed my own Word document to my insurance. You don't have to be totally
exact. No one can remember every detail. Just think back to any diet
programs you've been on and estimate the year and months, how much you
lost, and then how much you regained afterwards. Just round the numbers off
(like 30lbs, 20lbs). After thinking back, I had done some kind of diet
program (weight watchers, physicans weightloss, LA weightloss) about every
3 years and had gained every time afterwards. You'll be surprised at
yourself after you really look at things written down. Good Luck! Keep
after the insurance company and don't give up! Tell them you are determined
to get approved. Find out who your contact is and hound them to death. They
