WHEN YOU APPEAL DO YOU RE-SUBMIT EVERYTHING AGAIN?

or do you just send in appeal letters and medically neccesary letter from doctor, and all that is obtaining to appeal? Or does my surgeons office have to re submit everything? Thanks in advance....    — marie M. (posted on April 16, 2003)
