Question:
could someone help me do a diet diary for the insurance

My insurance requires a diet diary to be approved for surgery,I fixed one up and they called and left a mess. on my answering machine saying they needed more detailed history, Could some post ur diet history u done for ur insurance for me to use as a guide line to do mine? I included whether it was Dr supervised or not,date of diet, how long I was on it,how much I lost, what I regained.Leslie Elwood

Linda, Go to my profile. I have included the letter and health history (including Diet attempts) that I used for all my doctor appointments. Every doctor I saw really liked it and said it would help. I breezed right through approval and surgery in only a few months
