Hi.. My sister was approved by her insurer, went to the surgeon, had psch eval, then

The doctor can accept any patient he wants whether the insurance pays him or not. If the insurance doesn't pay him then the patient is responsible. Status between a doctor and an insurance company can change from day to day. Their only obligation is to inform you who is responsible for the bill before rendering services.
