Question:
Seeking a PhD thru Pacificare HMO in OKC that does WLS evals

I called many psychs and phd's in OKC and I cant get one to do my eval and bill Pacificare, they say Pacificare will not pay for it. Pacificare says they will if the doc uses the 90801 code, the doc wont, she says the WLS eval is much more intense than a reg eval and she wants $285 and Pacificare will only pay a third of that. I need a psych eval soon, everything else is going fine except that, I would appreciate any referrals to docs.    — Jane C. (posted on August 10, 2002)

I was reccomended Dr. Keller by my surgeon. I am not positive about which Insurance he takes or the regulations of Pacificare. His number is 405-942-4080.
